Product complaints
The following complaint conditions apply to returns from the EU countries, Switzerland and the UK. Products purchased from a distributor must be subject to complaints exclusively through the respective distributor (>).
Product complaints & warranty claims
The Zeramex® guarantee (>) applies to implants and secondary parts. If you wanto to submit a product complaint, please use the following complaint form.
Please note the following conditions:
- Please complete the form in full and attach all relevant x-rays and/or clinical images.
- The products concerned must be sterilised and individually packaged.
- Complaints must be submitted within 3 months of explantation with a fully completed complaint form.
- Incomplete complaints or complaints submitted later cannot be considered for replacement. If required information for mandatory fields is not available, please enter “n/a”.
